Adult Day Services:  Fees are based on 4 hours of care

 · Adult Day Health Care (ADHC) is a protective supervised daytime program providing skilled nursing, social services, and rehabilitative therapies, which includes recreational therapy, physical therapy, and occupational therapy for individuals with medical conditions and those at risk of requiring nursing home care . ADHC centers are reimbursed by Medi-cal and Veterans Administration.

 · Adult Day Program (ADP) is a non-medical adult day service, providing care to adults with physical and/or cognitive impairments who require personal care services, protective supervision or assistance in activities of daily living offers therapeutic activities tailored to the individual's abilities; nutrition services; basic health monitoring; transportation coordination; and respite and support for families. ADP centers are reimbursed through private pay.  Additional fees apply for Physical and Occupational Therapy.

Adopt-A Senior

This program helps to provide gifts to local seniors in need who wouldn’t otherwise receive any during the holidays. A member of the community can adopt a senior to provide holiday cheer. Please call for more information.

  • Enhanced Care  Management (ECM) program provides a coordinated approach to whole-person care that addresses the clinical and non-clinical needs of Partnership Healthplan members.

  • Traumatic Brain Injury (TBI)
    The Traumatic Brain Injury program is for adults, over the age of 18, who have been diagnosed with a TBI. This program is available to those in need of assistance in connecting with medical or social resources. Individualized goals will be set in collaboration with our TBI Navigators to help the participant reintegrate with the community in a healthy way. Access to community resources, training and support groups are opened to the individual once a need has been identified.

Foster Grandparent Program

This is a volunteer program where, as a Foster Grandparent, you are a role model and mentor, serving at local schools and youth facilities. You help children learn and read, provide one on one tutoring and guide children at a critical time in their lives. Please call for more information. Must be 55+ years old.

Senior Companion Program

As a volunteer (55+ years), you can become a volunteer and help a frail senior stay in their home. You can help by running errands or just be a friend which often times can help their caregiver with much need time off. Please call for more information.

Provides free counseling and information to help you understand your Medicare & Medicare Supplemental Insurance plans. HICAP does not write or sell insurance policies but will give the information about where to purchase a policy. If you are moving to the Redding area, and have Kaiser insurance, they can help with switching you from Kaiser to Medicare for your primary insurance or refer you to someone who can.

Geriatric care management, also known as Aging Life Care, provides support to at- risk older adults and their families. They connect them with resources, services and information in the community in order to help them stay independent in their own home. Care management services are helpful for those who have family out of the area.

The Gatekeeper Program: Educates the community on skills to identify vulnerable seniors and connect them with resources to help.
