This program helps frail elderly, who are at risk of placement in an institution, stay independent in their home. Each client will have a case manager to arrange assistance and monitor services. Some services arranged would include; meal preparation, transportation, shopping, physical or occupational therapy and much more! Must be 65 years or older and a MediCal recipient. PROGRAMS:
Geriatric care management, also known as Aging Life Care, provides support to at- risk older adults and their families. They connect them with resources, services and information in the community in order to help them stay independent in their own home. Care management services are helpful for those who have family out of the area.

The Gatekeeper Program: Educates the community on skills to identify vulnerable seniors and connect them with resources to help.

Cost: Free consultations. Resources, information and referrals about elder care issues are at no cost. Care management services are private pay (out-of-pocket).
